Résumé

Chez un patient souffrant d’algies périnéales, la névralgie pudendale est souvent évoquée, mais la névralgie clunéale inférieure, plus récemment décrite et moins bien connue, est une autre cause possible. Le diagnostic des deux affections dues à un syndrome canalaire repose sur des critères bien établis, dont quatre sont cliniques : une localisation évocatrice du territoire du nerf atteint, une douleur à la position assise, sans réveil nocturne ni déficit neurologique. La confirmation est apportée par le bloc anesthésique diagnostic. En l’absence de relevé de ces critères, l’incertitude diagnostique, encore trop fréquente, expose le patient à un retard de prise en charge ; il convient donc, 30 ans après la description initiale, de rappeler ces principes diagnostiques.

Abstract

Face to a patient suffering from perineal pain, the diagnosis of pudendal neuralgia is often evoked but the Inferior cluneal neuralgia, more recently described and less well known, is a possible cause of pain too. The diagnostic of these two pathologies is based on well-established criteria. Among them, 4 are clinical: location of pain in a specific territory, pain in the sitting position, without night wake up, and with no sensory deficit in the painful territory. The affirmation is brought by an anesthetic diagnostic block test. If these criteria are not well defined and analyzed, then the diagnosis remains uncertain and the therapeutic handling gets delayed. It appears to be important after 30 years of the initial description to point out these diagnostic principles.
